Our center now provides professional services for Faculty/Staff ID card replacement. In case of loss, damage, or the need to update information, please download the application form from our center's official website and complete all relevant information. The application process requires the applicant to visit our center in person and pay the replacement fee. The processing time is approximately 3-5 working days, and you will be notified via email for pickup. Service hours are Monday to Friday from 8:30 AM to 5:00 PM (closed on national holidays).

Instructions

  1.  Application fee: NT$150 per card

  2.  Application process:

    • Download the replacement application form (pdf).

    • Complete the application form and prepare NT$150 in cash (please bring exact change).

    • Visit the Information Technology Center on the 10th floor of Union Building I , Heping Campus II (No. 129-1, Section 1, Heping East Road, Taipei 106).

  3.  Processing procedure:

    • Our center will verify your information and produce the card.

    • Upon completion, the applicant will be notified via email (NTNU email) for pickup.
