Our center now provides professional services for Faculty/Staff ID card replacement. In case of loss, damage, or the need to update information, please download the application form from our center's official website, complete all relevant information and pay the replacement fee. The application process requires the applicant to visit our center in person. The processing time is approximately 3-5 working days, and you will be notified via email for pickup. Service hours are Monday to Friday from 8:30 AM to 4:30 PM (closed on national holidays, winter break, summer break and lunch break).

For lost cards: Download and complete the replacement application form (pdf) and the declaration of Loss form (pdf)
- For damaged cards: Download and complete replacement application form (pdf) and bring the old card.

Submit the completed forms and payment receipt to the Smart Campus Group, Information Technology Center on the 10th floor of Union Building I , Heping Campus II (No. 129-1, Section 1, Heping East Road, Taipei 106).(Ext. 3746).
